What to gather before you submit — and why each one matters.

You don't need everything to start — submit what you have and we'll tell you exactly what's missing. Each document below feeds a specific part of the A$495 + GST valuation report. Items marked required are needed for any matter; others are conditional or recommended.

    Member statement at the cohabitation date

    Required

    The opening balance for the roll-forward. Include the fund, member number and investment option held at that date.

    Member statement at separation (or today)

    Required

    The closing balance we reconcile to. Confirms the period and surfaces any unexplained gap in the records.

    Annual member statements across the relationship

    Required

    Year-by-year balances, contributions, rollovers, fees and insurance premiums — the spine of the reconstruction.

    Required

    Contribution and transfer history fills the gaps where fund statements are missing or partial — the basis of the Balance Benchmark Rating.

    Investment option / strategy history from the fund

    Conditional

    Tells us which return series to apply to each financial year. If unavailable, we substitute the default-strategy return and disclose it.
